I hereby authorize Hair of the Dog to conduct a home visit on a mutually agreeable date before an adoption decision is made.

I understand and acknowledge that at any point after the adoption it has been discovered that any statements made in this adoption application or to Hair of the Dog were false, Hair of the Dog has the right to annul the adoption and reclaim the adopted dog(s) from my home immediately.
